If you know of a child or young person that you think may benefit from Holding On Letting Go, please download and complete the enquiry form then email it or post it to the address below. Once we have received this form we will send a referral form to the parent/carer of the child/ren, which will need to be completed and returned to us in order for the family to be accepted onto the programme.
